Key Responsibilities

  • Conduct structured consultations in an independent capacity, managing patients with new issues or worsening chronic conditions.
  • Plan and execute patient home visits, addressing both urgent needs and chronic health concerns for housebound individuals.
  • Oversee the triage process managed by administrative staff, ensuring quality and training standards are met.
  • Collaborate with the nurse lead to optimize resource utilization.
  • Serve as a mentor and training resource for colleagues and trainees.
  • Collect and present data to facilitate service development.
  • Support the appointment system within the practice, functioning as part of a same-day service with a supervising physician for guidance.
