About the Role

We are seeking a skilled Community Case Manager to join our Amersham Adult Community Healthcare Team. As a key member of our team, you will be responsible for providing high-quality nursing interventions to patients in their home environment.

Key Responsibilities
  • Contribute to processes for proactively identifying patients who are high-intensity users of community and primary healthcare, and those at risk of unplanned admission.
  • Assess the physical and psycho-social needs of patients, instigating therapeutic treatments and care delivery based on best available evidence.
  • Improve health outcomes using specialist nursing skills and expertise.
  • Undertake referral management to develop and establish case management programmes for patients referred to the Adult Community Healthcare Team.
  • Manage a caseload of patients with long-term conditions who would benefit from case management.
