About the Role

We are seeking a highly skilled and dedicated Community Healthcare Specialist to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our Rapid Response & Intermediate Care service, you will play a vital role in providing expert interventions and rehabilitation to patients in their own homes.

Key Responsibilities
  • Provide comprehensive specialist assessment, review, and case management for patients and carers
  • Develop and implement personalized care plans to promote health gain and maximize independence
  • Collaborate with multidisciplinary teams to ensure seamless care delivery and coordination
  • Lead and manage the Rapid Response & Intermediate Care team to achieve excellent patient outcomes
  • Stay up-to-date with the latest clinical guidelines and best practices to ensure high-quality care

Requirements
  • Registered Allied Healthcare Professional with HCPC registration
  • Relevant postgraduate clinical training and experience in care of the elderly, orthopaedics, neurology, respiratory, and palliative care
  • Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team
  • Valid and current driving license and car to travel to locations throughout Buckinghamshire
