About the Role

Livewell Southwest CIC is seeking a highly skilled Registered Mental Health Nurse to join our Community Crisis Response Team. As a key member of our team, you will play a vital role in providing holistic care to patients in their own homes, aiming to avoid unnecessary hospital admissions.

Key Responsibilities
  • Assess, diagnose, and implement individualized care plans for patients with complex needs
  • Work collaboratively with multidisciplinary teams to provide comprehensive care and support
  • Develop and maintain strong relationships with patients, carers, and other healthcare professionals
  • Contribute to the development and implementation of clinical policies and procedures
  • Participate in reflective practice, clinical supervision, and appraisals to maintain professional expertise
Requirements
  • Registered Mental Health Nurse qualification and registration with the NMC
  • Experience in community-based care, preferably in a crisis response setting
  • Strong assessment and communication skills, with the ability to work autonomously
  • Ability to work flexibly, including evenings and weekends
  • Commitment to ongoing professional development and maintaining expertise in mental health nursing
