Job Summary
We are seeking a highly motivated and experienced Pharmacy Technician to join our expanding Hospital at Home service. This role offers an exciting opportunity to contribute to the delivery of safe and effective medicines management within a dynamic multidisciplinary team.
Main Duties and Responsibilities
- Provide technical support to pharmacists in the delivery of medicines optimisation services to patients receiving care at home.
- Collaborate with pharmacists, technicians, and other healthcare professionals to ensure continuity of care during patient transitions between hospital and community settings.
- Conduct medication reviews with patients, identifying potential issues and recommending appropriate interventions.
- Assist in the development and implementation of pharmaceutical care plans tailored to individual patient needs.
- Contribute to the discharge planning process by providing accurate and timely information regarding medications.
- Maintain accurate records and documentation in accordance with established protocols.
- Participate in ongoing professional development activities to enhance clinical knowledge and skills.
Essential Requirements
- Registration with the General Pharmaceutical Council (GPhC).
- Pharmacy Technician qualification (BTEC in Pharmaceutical Sciences or equivalent).
- Significant experience working as a pharmacy technician in hospital or primary care settings.
- Demonstrated understanding of current medicines optimisation principles and practices.
- Excellent communication, interpersonal, and problem-solving skills.
- Proficiency in IT systems and electronic health records.
- Valid driving license and access to a vehicle.
Desirable Requirements
- BTEC for Clinical Pharmacy Technicians.
- Experience in home visit settings.
- Training experience for healthcare professionals or unqualified staff.
