Job Summary
We are seeking a skilled and dedicated Social Worker to join our Adult Social Work Team in South Derbyshire. As a key member of our team, you will work closely with adults who have care and support needs, living in the community and in care placements, to complete person-centred assessments.
Key Responsibilities
- Carry out Care Act assessments and reviews for individuals with complex needs, arranging the provision of services to meet eligible needs.
- Work in a person-centred and strengths-based way with clients and their families to maintain and maximise their independence.
- Undertake personalised safeguarding enquiries, requiring strong communication and organisational skills.
- Assess capacity in accordance with the Mental Capacity Act.
- Act as duty worker on a rota basis, triaging new referrals and supporting clients and carers who require a more urgent response.
