About the Role

We are seeking a highly motivated and experienced Pathway Advisor to join our Leaving Care Team at Swindon Borough Council. As a Pathway Advisor, you will play a crucial role in supporting young people aged 16 to 25 who have been in our care, helping them to live independent lives and achieve their full potential.

Key Responsibilities
  • Develop and implement individualized Pathway Plans to address the changing needs of young people
  • Provide advice and support to young people, including guidance on education, employment, and life skills
  • Collaborate with other agencies and services to ensure a seamless transition for young people
  • Monitor and evaluate the effectiveness of Pathway Plans and make adjustments as needed
Requirements

To be successful in this role, you will need to be fluent in English and have a strong understanding of the needs of young people in care. You will also need to be able to work effectively in a team and build strong relationships with young people, their families, and other stakeholders.
