Job Duties

- Provide exceptional holistic and personalized patient care, aligning with the hospice's mission and values.

- Assess, plan, and implement a variety of interventions, regularly evaluating and reviewing care to ensure it meets the patient's evolving needs and promotes their independence.

- Collaborate with patients to establish shared goals, monitor progress, and adjust treatment plans using outcome measures to ensure the effectiveness of interventions.

- Offer high-quality non-pharmacological interventions to manage symptoms such as pain, breathlessness, anxiety, and fatigue.

- Provide guidance and support to patients and their families, friends, and caregivers as appropriate within your professional scope.

- Organize your schedule to work across various settings, including community outreach, inpatient units, and the Wellbeing Hub, while effectively managing your caseload and prioritizing tasks.

- Lead and contribute to the development of palliative rehabilitation programs tailored to patients' needs.

- Maintain awareness of treatment limitations and contraindications related to specific conditions, demonstrating skill in recognizing and managing palliative care emergencies.

- Apply a deep understanding of the implications of palliative care diagnoses when providing support and interventions.

- Communicate effectively, professionally, and compassionately with patients, caregivers, and colleagues throughout all stages of patient care.

- Utilize advanced communication skills to convey complex and sensitive information with empathy, managing distressing situations with care.

- Build therapeutic relationships with patients and their families, facilitating their active participation in informed decision-making.

- Ensure timely and appropriate referrals to other health or social services as needed.

- Collaborate with multi-professional teams to ensure safe and effective hospice admission and discharge processes.

- Lead, design, and facilitate group sessions and expert clinics within the Wellbeing Hub, such as breathlessness management services.

- Advocate for the principles and practices of physiotherapy within palliative care, extending the hospice's reach into the community and demonstrating the unique contributions of therapy to patient care.

- Maintain high standards of electronic patient documentation, including report writing, referrals, and assessments, in accordance with professional expectations.

- Adhere to hospice protocols for the ordering, administration, and documentation of clinical supplies and equipment.

- Collaborate effectively with Occupational Therapy colleagues to enhance patient care.

Qualifications and Experience

Essential

  • Proven experience as a Physiotherapist in both community and inpatient settings.
  • Background in Oncology and Palliative Care.

Desirable

  • Experience working with patients facing life-limiting conditions and diverse diagnoses, including frailty, neurological issues, and respiratory concerns.
  • Experience in leading group sessions.
  • Experience supervising junior staff.

Educational Requirements

Essential

  • Degree or Diploma in Physiotherapy.
  • Current registration with the Health and Care Professionals Council.
  • Specialized knowledge and application of physiotherapy assessment and interventions.

Desirable

  • Knowledge of non-pharmacological approaches to symptom management, including pain and breathlessness.
  • Recent qualification in Acupuncture or a willingness to pursue relevant training.
