About Us

Island City Network PCN is a collaborative network of healthcare professionals working together to enhance the health of our patient population. Our team includes Clinical Pharmacists, Pharmacy Technicians, Social Prescribing Link Workers, Care Coordinators, Physios, Paramedics, Practice Nurses, and Mental Health Facilitators.

Our management team is led by the Clinical Director and PCN Manager.

Job Responsibilities

The successful candidate will use available population data to identify patients with long-term conditions and work with them to ensure they receive the same level of care. They will also develop an understanding of local services available for patients with long-term conditions and signpost them accordingly.

The ideal candidate will have:

  • Ability to provide a culturally sensitive service, supporting people from all backgrounds and communities.
  • Commitment to reducing health inequalities and proactively working to reach people from diverse communities.
  • High level of written and verbal communication skills and the ability to communicate effectively with people, their families, carers, and other services.
  • Knowledge of and ability to work to policies and procedures including confidentiality, safeguarding, information governance, and health and safety.
  • Meets DBS reference standards and criminal records checks.
  • Willingness to work flexible hours when required to meet work demands.
  • Previous primary care experience working with SystemOne.
  • Knowledge of the services and teams that support patients within the community.
