Clinical Rehabilitation Specialist

We are seeking a highly skilled and experienced Clinical Triage and Rehab Support Practitioner to join our team at Your World Recruitment Group.
Job Summary:The successful candidate will be responsible for providing high-quality clinical triage and rehabilitation support services to our clients. This will involve assessing patients, developing and implementing rehabilitation plans, and working collaboratively with multidisciplinary teams to ensure effective care.
Key Responsibilities:- Provide professional and legal accountability for all aspects of clinical work.
- Work within codes of practice and professional guidelines.
- Undertake specialist holistic rehab assessments, including those with complex presentations and multi-pathologies.
- Control and manage triage calls effectively using skilled questioning, active listening, and excellent verbal communication skills.
- Evaluate patient progress and/or change, reassess, and understand the clinical rationale for altering a rehabilitation plan and interventions.
- Understand and be acutely aware of significant medical observations during assessment and treatment of patients and know who to report this to.
- Complete written and thorough assessments of all clients requiring an in-person assessment.
- Work with the business development team, admissions, and discharge coordinator to share findings of assessments in a proactive and timely manner.
- Maintain effective and efficient communication lines with HOCS to discuss complex clients or assessments.
- Rotate into clinical teams as appropriate to support client rehab and/or care and maintain clinical skills.
- Be responsible for the handover process to relevant QEF CRC clinical staff regarding outcome of clinical triage and/or face assessment.
- Demonstrate the importance of gaining patient and where appropriate family, carer, or additional professional consent for all interactions and assessments.
- Effectively communicate with patients, family members, carers, and colleagues using tact and persuasive skills.
- Work in line with QEF policies and procedures.
- Work within your own professional body, demonstrating a good working knowledge of national and local standards of practice.
- Organise and manage own time in line with line manager expectations.
- Conduct and contribute to risk assessments and relevant health and safety assessments.
- Be responsible for the handover process to relevant QEF CRC staff of resulting recommendations from risk and health and safety assessments.
- Take an active part in the development of the QEF CRC service and in supporting organisational aims and objectives.
- Work effectively within the service and liaise appropriately with members of the MDT.
- Ensure accurate and timely written records are kept, complying with QEF policy and HCPC guidance.
- Participate in the on-going development of the role and support the development of protocols and processes in the Triage.
- Support the HOCS, General Manager, and therapy team leads with day-to-day planning and delivery of a high-quality service.
- Participate in annual audit plans for the Service and annual staff surveys.
- Undertake evidence-based practice to further own clinical practice and maintain competency.
- Comply with the collection of data required by the service, including SSNAP data.
- Participate in clinical supervision.
- Participate in the staff appraisal scheme and personal development plans.
- 5+ years post-qualification clinical experience.
- 3+ years community experience.
- 3+ years experience working with neurologically impaired adults.
- Solid experience of working in a team/MDT environment.
- Experience of working in a rehab setting.
- Experience of working autonomously.
- Experience of carrying a caseload.
- Experience of making decisions based on clinical rationale.
- Experience of working in a customer-focused environment.
- Registered nurse or postgraduate physiotherapist/occupational therapist.
- Current NMC or HCPC registration.
- Completed Mentorship and/or clinical supervision training.
- Evidence of ongoing professional development.
- Postgraduate training (MSc, MSc modules, PGDip, PGcert).
