Health Visitor

Hunter AHP is seeking a dedicated and compassionate Health Visitor to join our team of healthcare professionals.
In this role, you will play a vital part in promoting the health and well-being of families and individuals within the community. You will work collaboratively with multidisciplinary teams to deliver high-quality care and support.
Your responsibilities will include:
- Managing a caseload of patients and providing comprehensive health assessments.
- Delivering the Healthy Child Programme and other health interventions.
- Working in partnership with families, carers, and other professionals to develop care plans.
- Advocating for the needs of children, families, and communities.
- Contributing to the development and implementation of health policies and initiatives.
We are looking for a candidate who possesses:
- A SCPHN Health Visitor Post Graduate Diploma or Certificate.
- Current NMC Registration.
- Excellent communication and interpersonal skills.
- Proven experience in multi-agency working and child protection.
- A commitment to ongoing professional development.
- Knowledge of current health agendas relating to children, families, and health visiting.
