Position Overview

The Care Coordination Specialist will engage directly with clients, caregivers, service providers, and care management personnel while orchestrating and negotiating care packages to ensure optimal support while maintaining cost-effectiveness.

This role is crucial in facilitating the prompt and precise generation of service contracts and, when necessary, offering guidance and support to members of the Social Care Team.

Key Responsibilities

  • Process care service requests from care management teams through the designated system.
  • Clarify service needs with care management personnel, clients, and providers to ensure mutual understanding of expectations.
  • Cultivate a comprehensive understanding of all available resources to address the needs of clients, including both commissioned and community services.
  • Generate individual service contracts for all arranged services.
  • Review existing contracts to ensure they are concluded, voided, or modified in a timely and appropriate manner.
  • Participate in shared team responsibilities as dictated by service needs.
  • Ensure clients receive the most suitable support promptly while adhering to contractual obligations.

About Our Organization

The Care Coordination Team is a newly established function within the Trust, tasked with researching and compiling a variety of personal support options to foster independence and enhance well-being in alignment with the model of care.

Qualifications and Training

Essential

  • NVQ Level 4 in business or administration or equivalent experience.
  • GCSEs in English and Mathematics.
  • Proficient advanced IT skills.
  • Evidence of literacy and numeracy achievements.

Desirable

  • Demonstrated consistent learning through education, training, and experience.

Knowledge and Experience

Essential

  • Understanding of care management processes and contractual principles.
  • Familiarity with the Care Act.
  • Awareness of the care provision market.
  • Strong negotiation skills.
  • Accurate recording and reporting abilities.
  • Capacity to prioritize workload effectively.
  • Proficient in computer software such as Excel and Word.
  • Experience working in a high-pressure environment.

Desirable

  • Familiarity with the Trust's processes and IT systems.
  • Understanding of the Trust's contracting systems.
  • Knowledge of the local care market.

Specific Skills

Essential

  • Proven experience in public/client interaction.
  • Excellent communication skills with the ability to convey complex contractual information to non-financial staff and managers.
  • Strong organizational skills and the ability to handle incoming information confidentially and sensitively.

Desirable

  • Experience with financial systems and care management software.
  • Background in social or health care settings.

Work Environment Requirements
