Key Responsibilities- Work collaboratively with our District Nursing Team to provide evidence-based care to patients in their own homes or in a clinic setting.
- Identify changes in patients' conditions and communicate these to the team leader, ensuring timely interventions and adjustments to care plans.
- Practice in a safe and confident manner, adhering to principles of health, safety, and security at all times.
- Provide support and advice to patients and their carers, promoting self-care and independence through rehabilitation programs.
- Assist in providing treatments and interventions for patients, including phlebotomy, urinalysis, patient height and weight measurement, blood pressure monitoring, temperature and pulse recording, wound care, blood glucose monitoring, and swab samples.
- Participate in health promotion activities and the prevention of ill health and disability in the community.
- Team player with a flexible approach to work.
- Motivated and enthusiastic individual with a willingness to learn and develop skills.
- Knowledge of Primary Care and the role of Community Nursing, as well as Integrated Care Teams and the voluntary sector in the community.
- NVQ 3 or equivalent qualification, with a willingness to undertake further training and expand role within the District Nursing Team.
- Good communication skills, IT skills, and ability to take blood and other samples or willingness to learn.
- Able to work flexibly across a range of disciplines and conduct patient monitoring activities, take and record blood pressures, pulse and temperature, blood sugar monitoring, and understand principles of wound care.
