We are seeking an experienced Advanced Clinical Pharmacist to join our team at Primary Care 24, a leading social enterprise delivering NHS contracts exclusively. As a key member of our organization, you will play a crucial role in developing and delivering our medicines optimization and safety agenda.
Key Responsibilities- Develop and implement prescribing policies and procedures to ensure safe and effective medication use.
- Conduct clinical medication reviews with patients to optimize their medication regimens and improve health outcomes.
- Manage the repeat prescribing reauthorization process to ensure timely and accurate medication dispensing.
- Work with care home staff to improve the safety of medication ordering and administration.
- Reconcile medications following discharge from hospitals and intermediate care to prevent medication errors.
- Degree in Pharmacy and Independent Prescriber V300 qualification.
- Experience working in urgent care and/or primary care settings.
- Minimum of 2 years post-qualification experience.
- In-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare principles.
- Ability to work at pace in a busy working environment and multitask effectively.
