About the Role

We are seeking a highly skilled and dedicated Mental Health Specialist Practitioner to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with severe and enduring mental health needs.

Key Responsibilities
  • Undertake comprehensive mental health assessments in a wide range of situations and locations, using standardized assessment tools such as the Cluster Allocation Support Tool (CAST) and the Recovery Star.
  • Develop and deliver complex treatment programs using evidence-based practice, including strategies to manage risk for service users with complex needs and carers.
  • Act as a care coordinator for service users, providing defined interventions to individuals on other workers' caseloads.
  • Deliver a range of defined activities/interventions to improve carers' ability to support service users and enable them in their relationship with the service user.
  • Develop and maintain good partnership working with other services throughout all treatment episodes, including regular liaison with Primary Health Care Teams, inpatient and intensive services, day services, and the voluntary sector.
  • Facilitate the development of comprehensive crisis plans, rapid access plans, and advance statements, involving other agencies as necessary.
  • Build and lead hope-inspiring relationships with service users, acknowledging their personal journey and focusing on strengths and aspirations to create meaningful personal recovery plans.
  • Maintain own workload and plan the workload of others, ensuring effective time management and utilization of electronic resources such as diaries/scheduling.
  • Protect individuals from abuse and harm in line with local safeguarding policies and procedures, including working with others to plan, develop, and review protection plans.
  • Coordinate and undertake protective interventions as outlined in protection plans, taking individual action where necessary.
  • Collaborate with individuals with a range of mental health needs to develop skills to manage their own health, in accordance with their personal recovery plan, using approaches that are affirming, build on strengths, identify past positive experiences, and use small steps to move towards their goals.
  • Undertake active clinical leadership and supervision within the team, covering for the manager and colleagues as required.
  • Maintain the single health and social care record, ensuring both paper and electronic records are up to date in accordance with professional and organizational standards.
  • Report and record within agreed timeframes all activity relating to information reporting and performance requirements.
  • Participate in management, caseload, and clinical supervision in accordance with trust policy.
  • Provide mentoring/training for others in relevant practice areas, taking a collaborative approach to practice development and evidence-based care.
  • Demonstrate responsibility for developing own practice in line with professional qualifications and contributing to the development of others, using effective feedback, supervision, coaching, and appraisal.
  • Adhere to professional codes of conduct, ensuring required skills and competencies are maintained.
  • Monitor and maintain health, safety, and security of self and others, undertaking assessments and taking appropriate action where necessary.
  • Develop own knowledge and practice, and contribute to the development of others, making use of available feedback, supervision, and appraisal to identify areas for development and accessing learning and training opportunities.
  • Lead, maintain, and participate in practices that enable effective team working.
Person Specification
  • Essential:
    • Diploma level/Degree in a relevant health/social care profession, e.g., RMN, OT, Social Worker.
    • Current relevant registration with the NMC or HCPC and commitment to CPD.
  • Desirable:
    • Relevant post-registration qualifications, such as Thorn, Non-Medical Prescribing, CBT, AMHP, Brief Interventions, etc.
Experience
  • Essential:
    • Demonstrates substantial post-registration experience gained working in a range of frontline adult mental health services, undertaking assessment and delivering interventions.
  • Desirable:
    • Experience gained in a wide variety of settings.
Skills
  • Essential:
    • Highly developed verbal communication skills, able to engage effectively with people at all levels, even when a more assertive approach is needed.
    • Able to deliver a range of key therapeutic interventions, including CBT, Psychosocial interventions, Family Interventions, and DBT approaches.
