We are seeking a qualified and experienced Senior Social Worker to join our Adults Assessment Team in Wokingham Borough Council.
About the RoleThis is a hybrid role, with Tuesdays spent in the office, where you will be responsible for investigating referred cases, assessing the needs of individuals, and making recommendations on eligibility for health funding.
You will work collaboratively with a range of care workers and clinicians to assess complex cases and provide recommendations on funding splits within agreed frameworks and eligibility criteria.
Key Responsibilities- Investigate referred cases and assess the needs of individuals
- Make recommendations on eligibility for health funding and proposed funding splits
- Work collaboratively with care workers and clinicians to assess complex cases
- Support the Team Manager on operational issues to ensure high levels of team and individual performance
- Design and deliver training across the council and health partners on CHC, S117, Continuing Care, and other funding streams
To be considered for this role, you must have a social work qualification, full SWE registration, and over 3 years of post-qualified experience gained in an Adults Social Services setting.
You will be required to work in a hybrid environment, with Tuesdays spent in the office, and will be expected to support the Team Manager on operational issues.
