Join Our Team as a Case Management Associate

We are seeking a highly skilled and compassionate Case Management Associate to join our team at Social Return Case Management. As a Case Management Associate, you will play a vital role in providing comprehensive case management services to clients with complex conditions in the United Kingdom.

About the Role

As a Case Management Associate, you will work closely with our team to provide personalized support to clients with acquired brain injury, spinal injury, cerebral palsy, serious orthopaedic injury, and prolonged disorders of consciousness. Your expertise will be essential in developing and implementing effective case management plans that promote optimal outcomes for our clients.

Key Responsibilities
  • Conduct thorough assessments of clients' needs and develop tailored case management plans
  • Collaborate with healthcare professionals, families, and caregivers to ensure seamless care coordination
  • Provide education and support to clients and their families on managing complex conditions
  • Monitor and evaluate the effectiveness of case management plans and make adjustments as needed
Requirements

To be successful in this role, you will need:

  • Experience working with clients with complex conditions, particularly acquired brain injury, spinal injury, and cerebral palsy
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
  • Ability to work independently and as part of a team, with a high level of autonomy and flexibility
  • Strong analytical and problem-solving skills, with the ability to think critically and creatively
