Position Overview
The Barking, Havering and Redbridge University Hospitals NHS Trust is addressing the distinctive challenges presented by an aging demographic within the community. With a significant portion of the population aged 65 and older, there is an increasing necessity for effective, coordinated healthcare solutions tailored to older adults.
We recognize that delivering high-quality care to this population can be complex. There is a pressing need for innovative strategies to enhance access to geriatric expertise and minimize avoidable hospital admissions, as such admissions can lead to adverse outcomes including deconditioning, infections acquired in hospitals, malnutrition, delirium, loss of independence, and psychological effects.
Our organization is dedicated to transforming the care model for our growing elderly and frail population by adopting a proactive approach. This involves identifying individuals at risk of frailty and implementing early interventions aimed at improving management, preventing deterioration, and potentially reversing frailty. Central to this initiative is a comprehensive geriatric assessment conducted by a multi-disciplinary team.
Key Responsibilities
We are enhancing our efforts to manage patients who may be approaching a healthcare crisis that could lead to hospital visits. This includes offering urgent assessment appointments on the same or next day and providing a limited number of infusion chairs for patients requiring short-term intravenous therapy.
All services will be delivered from our new Health and Wellbeing Hub, which is set to be a multi-provider facility. In addition to the frailty unit, this hub will encompass primary and community care, diagnostics, and social services, with the ambition to significantly improve care delivery for the aging population.
