Key Responsibilities
  • Lead on the assessment of needs for individuals referred to our projects.
  • Establish individualized support plans for each person, working collaboratively with other relevant organizations.
  • Provide resettlement support when individuals move to new accommodation.
  • Keep accurate records of all support provided.
Requirements
  • Experience of supporting individuals with mental health and wellbeing issues.
  • Excellent understanding of housing and homelessness.
  • Knowledge of trauma-informed practice, attachment theory, and relationship-based approaches.
  • Good understanding of safeguarding vulnerable adults.
  • Ability to work with multi-agencies effectively.
  • Highly organized, resilient, compassionate, and non-judgmental.
What We Offer
  • A full or part-time position, depending on your requirements.
  • Permanent or fixed-term contract, depending on the position and funding.
  • Salary of £23,088 per year, pro-rata for part-time.
  • 45p per mile travel expenses for work-related mileage.

Please note that access to a vehicle is a requirement for this role, as it covers the Swansea area. A DBS on the update service or the ability to carry out a new DBS check is also essential.
