About the Role

We are seeking a skilled and compassionate Care Support Worker to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a Care Support Worker, you will play a vital role in providing high-quality care and support to patients in a community setting.

Key Responsibilities
  • Assess and monitor patient conditions, identifying any changes or concerns and escalating as necessary.
  • Deliver patient care as directed by the Registered Nurse, ensuring the highest standards of care and compassion.
  • Work collaboratively with the healthcare team to develop and implement individualized care plans.
  • Provide emotional support and reassurance to patients and their families, promoting a positive and respectful environment.
  • Contribute to the maintenance of a clean and safe environment, adhering to infection control and health and safety protocols.
Requirements
  • A minimum of 6 months paid experience in a community environment within the last 2 years.
  • Comply with NMC Professional Registration and keep up to date with current nursing practices.
  • A valid UK driving license and access to a car.
