About the RoleAs a care co-ordinator, you will be responsible for providing support and care to a wide range of clients. You will need to have experience in supporting and caring for all ages and stages of illness or recovery.
Key Responsibilities- Plan and manage day-to-day rotas for carers, including re-planning work schedules to cover sickness or emergencies.
- Write and update care plans, and maintain client information on our digital software systems.
- Liaise with clients, their families, and other related contacts to support the needs of our service users.
- Provide domiciliary care as and when required by the business, including evenings, weekends, and bank holidays.
- Cover the on-call phone service, which may involve working weekends in exchange for the next two days off.
- Responsible for medication ordering and reviewing with carers.
- Perform other tasks as required to support the business, our service users, and colleagues.
To be successful in this role, you will need to have a NVQ Level 3/Diploma in Health & Social Care qualification and experience using CarePlanner or a similar system.
A full clean driving licence and your own car are also essential, as you will need to travel to visit clients and attend meetings.
