Crisis Support Worker

We are seeking a skilled and compassionate Crisis Support Worker to join our team at Bradford Council. As a Crisis Support Worker, you will play a vital role in delivering one-to-one crisis support to individuals in mental health crisis, providing a safe and supportive environment for them to work through their feelings and access the support they need.

Main Responsibilities:
  • Deliver one-to-one support sessions to individuals in mental health crisis, using a recovery-focused and person-centred approach.
  • Manage referrals, book appointments, and conduct follow-up calls after crisis sessions.
  • Make onward referrals and signpost individuals to relevant services.
  • Assist in risk assessments with multi-disciplinary staff, highlighting any changes in individuals' presentation relevant to their safety plan.
  • Participate in shift handover and team meetings.
  • Offer outreach sessions within the community, such as at care homes, community centres, schools, and in individuals' homes.
  • Liaise with professionals and organisations across the district, including referrers and partners.
  • Ensure that client notes and information are recorded in an accurate and timely manner and kept updated.
