About the Role

Location: Community-based across the East Cheshire region with homeworking elements.

As a Dementia Support Specialist, you will have the rewarding opportunity to provide support, information, and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives.

The service is unique to each individual as it is based upon their personal circumstances and support needs. You will offer support to your clients in a variety of ways, whether in their own home, other locations in the community, face-to-face, or through remote communication.

You will also assist people to access other services, including our own programs, providing signposting and referrals.

About You

  • Ability and interest in supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives.
  • Understanding of dementia and the needs of those living with dementia and their carers would be an advantage, but if you have had experience supporting individuals with other diagnoses or support requirements, this would be taken into consideration.
  • Experience with communicating with a wide range of people, adapting your approach to ensure understanding at all levels.
  • Able to network effectively and collaborate with other professionals to achieve a positive outcome for people.
  • Ability to work effectively and collaboratively as part of a local and regional team, while also demonstrating strong independent work skills.
  • Good IT skills, experience working with databases and virtual meeting software.
  • The ability to travel across the local area independently when required.
