We are seeking to appoint a Level 2 or Level 3 Community Support Specialist

As a member of the Dewsbury Community Wellbeing Team, you will conduct initial assessments, reviews, and safeguarding inquiries for vulnerable individuals, collaborating with their informal and formal caregivers within the community. Depending on your experience, you may also engage in court-related work.

Your focus will be on adults aged 18 and over who have physical and sensory disabilities, as well as mental health needs. A strong, customer-centered approach is essential to alleviate the distress experienced by service users and to ensure positive outcomes.

You will participate in multi-agency collaboration with various health and social care professionals to secure the best results for individuals. A solid understanding of operational practices, legal frameworks, and evolving agendas impacting partners, particularly within the NHS, is crucial.

Your role will involve working with adults and caregivers to promote Self-Directed support and Self-Assessment, making eligibility determinations as outlined in the Care Act (2014), along with safeguarding responsibilities. Utilizing outcome-focused assessments and knowledge of the Mental Capacity Act, you will adopt a proactive approach to risk management.

Our team fosters a supportive environment; we have approachable managers who maintain an open-door policy, conduct regular advice sessions, and are receptive to feedback. We work collaboratively to support one another and share our expertise.

We are looking for a Community Support Specialist to join our team and thrive in a flexible and supportive setting at Level 2 or Level 3.
