About the Role:

We are seeking a dedicated and compassionate Care Coordinator to join our team at Pennine Care NHS FT. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals with a learning disability, aged 16-25, who are experiencing a period of crisis.

Key Responsibilities:

  • Provide care and support to individuals with a learning disability, promoting their independence and well-being
  • Work as part of a multidisciplinary team to deliver therapeutic activities and group work
  • Develop and maintain effective therapeutic relationships with service users, families, and carers
  • Participate in the development and implementation of Positive Behaviour Support Plans
  • Assist with daily living tasks, such as personal hygiene, toileting, and dressing
  • Support service users at mealtimes and participate in education and training sessions

Person Specification:

  • NVQ level 3 in Care (or equivalent experience) or a commitment to work towards
  • Care Certificate or willing to work towards
  • Experience of working with children and young people, individuals with a learning disability, and/or autism
  • Experience of providing care and support in a community setting and/or residential setting
  • Ability to remain calm in stressful situations and display patience and empathy
  • Ability to communicate effectively with clients and their carers

Skills and Abilities:

  • Ability to impart knowledge, supervise, and direct the work of others under supervision
  • Ability to develop and maintain effective therapeutic relationships with service users, families, and carers
  • Ability to organise day-to-day tasks and activities based on an agreed work plan
  • Ability to work a flexible shift pattern over 7 days, including the rotation of night duty

Work Related Circumstances:

  • Ability to work in a changing environment and demonstrate an interest in working with adults and CYP
  • Resilient and able to work within a changing environment
  • Ability to use a car or access to a means of mobility to travel across the Trust footprint in line with service needs
