About the Role

We are seeking an enthusiastic and self-motivated individual to enhance our clinical team at Aelfgar Surgery. Our team provides high-quality patient care, and we are looking for a skilled Advanced Clinical Practitioner to join us.

Key Responsibilities
  • Deliver high-quality care to patients, including initial presentation, assessment, diagnosis, treatment, and evaluation.
  • Work closely with our team members to deliver a minor illness service to our patients.
  • Attend to patients who wish to be seen by a healthcare professional, diagnose and treat patients presenting with minor illness, and advise people in respect of their continuing medical and nursing needs.
  • Prioritize and triage the needs of patients, making necessary referrals for investigations and acting upon the results of those investigations.
Requirements
  • Professional registration with the Nursing and Midwifery Council (NMC) or Health Care Professions Council (HCP).
  • MSc in Advanced Clinical Practice or equivalent.
  • Previous primary care experience, including management of minor illness, minor ailments, and injuries.
  • Evidence of Continuing Professional Development (CPD) in autonomous practice.
  • Ability to evaluate safety and effectiveness of own practice.
  • Chronic disease management experience.
  • Experience in telephone triage and visiting patients in their own home or residential setting.
Desirable Qualifications and Experience
  • Teaching and mentoring experience and/or qualification.
  • Independent prescribing or study of pharmacology and therapeutics at postgraduate level.
  • Supervisory and management experience.
  • Experience of clinical audit, evaluation research, and implementation of evidence-based practice.

  • A sound understanding of the national and local health agenda.
  • Understanding of evidence-based practice.

  • Ability to assess and manage patient risk effectively and safely.
