We are seeking an experienced Registered Nurse or HCPC Paramedic to join our team as a Community Practitioner in our Urgent Community Response Team (UCRT). This is a challenging and rewarding role that requires a high level of clinical expertise and a passion for delivering patient-centered care in a community setting.
Key Responsibilities- Undertake rapid assessments and develop treatment plans for patients in crisis
- Work collaboratively with other healthcare professionals to deliver holistic care
- Provide leadership and support to the team, including leading on projects and coordinating clinical activities
- Develop and maintain strong relationships with patients, families, and carers
- Registered Nurse Part 1,2 NMC Register or HCPC Paramedic Registration
- A minimum of 3 years current experience practicing as band 5 in the Community/Acute environment
- Consultation & Diagnostic qualification or working towards
