Position: Lead Clinical Auditor for Continuing Care Assessments

Type: Fixed Term / Secondment

Hours: Full Time, 37 hours per week

Salary: £45,217 to £54,395 per annum

Location: Community-based

Role Overview:

The Lead Clinical Auditor for Continuing Care Assessments will be responsible for delivering a comprehensive and high-quality internal clinical audit and advisory service within the Council. This role will involve collaboration with key stakeholders, including Social Workers, to foster a culture of continuous improvement. The focus will be on supporting Care Plans for Children with Disabilities, ensuring that care packages are effectively arranged and funded in partnership with the NHS and Local Authority.

Key Responsibilities:

  • Collaborate with health and social care stakeholders to provide expert clinical advice on individual cases and appeals, ensuring compliance with statutory requirements.
  • Assist Social Workers in completing the Continuing Care Check List for new cases, offering guidance to uphold the highest standards.
  • Effectively communicate complex and sensitive information regarding cases to facilitate appropriate applications of the Continuing Care Checklist.
  • Provide clinical guidance at Multi Agency Resource Forums and other panels, ensuring comprehensive assessments and eligibility determinations for Continuing Healthcare and Funded Nursing Care.
  • Establish strong working relationships across the Council and its partners, staying informed on clinical developments and supporting continuous improvement initiatives.
  • Participate in service enhancement activities, including auditing cases where Continuing Care has been declined, to identify and mitigate future risks.
  • Conduct regular audits and reviews of care plans to assess service performance and provide actionable insights for improvement.
  • Adhere to relevant legislation and professional standards to ensure efficient use of public funds and high-quality service delivery.
  • Maintain current knowledge of legislation and Department of Health guidance to ensure policies and procedures are effective and relevant.

Qualifications and Experience:

  • Registered Nurse (RGN/RMN) with experience in a multi-agency setting.
  • Knowledge of assessment processes for NHS Continuing Healthcare and Funded Nursing Care.
  • Proven ability to develop partnerships and collaborate with multiple stakeholders across the health sector.
  • Strong organizational skills with a track record of managing diverse workloads in dynamic environments.
  • Self-motivated team player committed to delivering results and capable of working autonomously.
  • Excellent communication skills with the ability to translate complex information into actionable insights.
  • Membership with the Nursing and Midwifery Council and evidence of ongoing professional development.

Mobility Requirements:

The role necessitates mobility across a wider operational area, including the ability to transport service users and their belongings as needed.
