Our team has received national recognition, being finalists in prestigious awards and winning accolades for our Integrated Discharge Team's efforts in quality improvement and innovation.
We currently have an opening for a Social Worker within our Hospital Social Work Team, focusing on Community Rehabilitation Beds as part of our Care Close To Home Strategy. This position offers opportunities to engage with Rehabilitation Services, Continuing Health Care, and Hospice care, all within an agile working environment.
As a member of a multi-disciplinary team, you will support individuals discharged to Rehabilitation beds, facilitating their transition through timely strength-based assessments and planning that fosters independence, choice, and autonomy.
Your responsibilities will include implementing and promoting safeguarding procedures, adhering to departmental recording policies, and participating in multi-agency discharge planning meetings to ensure timely transitions. You will also provide support to carers and family members, offering guidance and conducting Carers Assessments in accordance with the Care Act 2014.
About You:

- A recognized social work qualification and be registered with the Social Work England (SWE), or
- Be a newly qualified Social Worker eligible for a fully supported ASYE year.
You should thrive in a dynamic environment and be passionate about promoting individuals' strengths while ensuring a safe approach to social work practice. Successful candidates will hold up-to-date professional registration and demonstrate experience in person-centered and strengths-based approaches.
Additionally, you will need to exhibit the ability to work collaboratively within a team that prioritizes the strengths of individuals, families, and communities in care and wellbeing. Participation in the Out of Hours (OOH) stand-by duty rota may be required on an occasional basis.
