Social Worker

We are seeking a skilled and dedicated Social Worker to join our Hospital Social Work Team, supporting individuals who have been discharged to Rehabilitation beds. As a key member of our multi-disciplinary team, you will work closely with colleagues to promote timely discharges, strength-based care planning, and the maintenance of independence, choice, and autonomy for service users.
Key Responsibilities- Implement and promote departmental safeguarding procedures and adhere to departmental recording policy
- Attend multi-agency discharge planning meetings to ensure timely discharges
- Support carers and family members, sharing and advising where required, and offering Carers Assessments in line with the Care Act 2014
- Work as part of a team that puts the strengths of individuals, families, and community at the heart of care and wellbeing
- Social work qualification and SWE registration
- Ability to work in a fast-paced environment and promote a person-centred and strengths-based approach
- Proven ability to work as part of a team and demonstrate skills and experience of promoting a person-centred and strengths-based approach
- Knowledge and understanding of The Care Act 2014, Mental Capacity Act 2005, Data Protection Act 1998, and evidence of ability to work to practice guidance/procedure
