Our team has been recognized nationally, being finalists in prestigious awards and winning the Isle of Wight's NHS Quality Improvement and Innovation Award as part of the Integrated Discharge Team, which has received significant acclaim.

We are currently seeking a Social Worker to join our Hospital Social Work Team, focusing on Community Rehabilitation Beds as part of our Care Close To Home Strategy. This position offers opportunities to engage with Rehabilitation Services, Continuing Health Care, and Hospice care, all within an agile working environment.

As a member of a multi-disciplinary team, you will support individuals discharged to Rehabilitation beds, facilitating their transition through timely strength-based assessments and planning that prioritize independence, choice, and autonomy.

Your responsibilities will include implementing and promoting safeguarding procedures, adhering to departmental recording policies, and participating in multi-agency discharge planning meetings to ensure efficient discharges. You will also provide support to carers and family members, offering assessments in accordance with the Care Act 2014.

  • Hold a social work qualification and be registered with SWE, or
  • Be a newly qualified Social Worker, eligible for a fully supported ASYE year.

You should thrive in a dynamic environment and possess a passion for promoting individuals' strengths while ensuring a safe approach to social work practice. Successful candidates will have the necessary qualifications and maintain an up-to-date professional registration.

Demonstrated experience in promoting a person-centered and strengths-based approach is essential. You will be expected to work collaboratively within a team that prioritizes the strengths of individuals, families, and the community in care and wellbeing.

Knowledge of relevant legislation, including The Care Act 2014 and the Mental Capacity Act 2005, is crucial. You may also be required to participate in the Out of Hours (OOH) stand-by duty rota as needed.
