Job Summary

We are seeking a highly organized and efficient Care Navigator to join our friendly and patient-focused team. This is a permanent part-time position, working 25 hours per week, with shift patterns to be confirmed.

Main Responsibilities
  • Provide exceptional patient care and support, working closely with our multi-professional Primary Care Team.
  • Prioritize tasks, manage workload, and demonstrate adaptability in a fast-paced environment.
  • Develop strong relationships with patients, colleagues, and external partners, promoting a culture of co-operation and productivity.
  • Utilize electronic communications and computer systems to maintain accurate records and deliver timely information.
  • Maintain confidentiality, attention to detail, and a commitment to delivering high-quality services.
Person Specification
  • Organizational skills, with the ability to prioritize tasks and manage workload effectively.
  • Excellent communication and interpersonal skills, with the ability to work with people of all backgrounds.
  • Computer literacy and experience with electronic communications.
  • Ability to work in a fast-paced environment, with a high level of adaptability and flexibility.
