Care and Treatment Review/DSR Coordinator

We are seeking a highly motivated individual to coordinate both our Dynamic Support Register and our Care (Education) and Treatment Reviews within the Bradford district, specifically, children with a learning disability and/or autism.
Key Responsibilities- Hold the Care, Education and Treatment (CETR) register for Bradford District Care Foundation Trust.
- Arrange and coordinate CETRs in Bradford (Children's Services), alongside the Transforming Care Coordinator.
- Coordinate Dynamic Support Register (DSR) and chair the meetings.
- Provide education and training around CETRs and DSR for adult services within BDCFT.
- Provide data and information to meet national reporting requirements and in support of the Transforming Care Partnership Board via a monthly CETR report.
- Evidence of education at degree level.
- Experience in health/social care around DSR and CTR.
- Learning disability/mental health or social care experience around CTR/DSR.
- Significant experience in specialist area.
- Detailed knowledge of mental health legislation and other relevant policy and statutory guidance.
- Significant knowledge and experience of working with people with learning disabilities and autism.
- Experience of working within forensic settings, CAMHS services, CMHT's and/or learning disability services.
- Experience of planning with families and people with learning disabilities and/or autism.
- Car driver with own vehicle.
