We are seeking a highly motivated and experienced Assistant Practitioner to join our Mental Health Liaison Team at Southern Health NHS Foundation Trust.
Job SummaryThe successful candidate will work closely with our multidisciplinary team to provide high-quality care and support to individuals with mental health needs and learning disabilities. This is a permanent position, working 27 hours per week, with rotation of shifts including nights.
Main Responsibilities- Undertake the role of a keyworker for individuals with mental health needs and their carers, providing support and guidance in the Emergency Department and acute wards.
- Triage and diversion of individuals presenting with mental health crises, ensuring their physical health needs are met as appropriate.
- Support individuals presenting in mental health crisis, or with serious mental illness, dementia, or learning disabilities, and provide support to their carers, relatives, and friends.
- Work collaboratively with the service user and their family, friends, or carers, using knowledge and skills to work in a recovery-focused way, developing and reviewing appropriate care plans.
- Support effective communication between the Emergency Department, wards, mental health crisis team, and liaison service, ensuring all referrals are assessed within one hour.

Person Specification- NVQ Level 3 in Health and Social Care or equivalent.
- Experience of using IT packages, working within a team environment, and working alongside people with mental health issues and vulnerable service users.
- Previous experience of working in the NHS or healthcare sector.
