We are seeking a skilled and compassionate Mental Health Complex Case Manager to join our team at East Lancashire Hospitals NHS Trust. As a key member of our Integrated Discharge Service, you will play a vital role in providing proactive and efficient case management for adult mental health patients across all ELHT hospital sites.
Key Responsibilities- Provide clinical advice and guidance to professionals within the Integrated Discharge Service.
- Support patients with psychological and mild to moderate mental health needs associated with long-term physical conditions.
- Conduct assessments required for discharge and develop robust care plans in conjunction with patients and wider multidisciplinary teams.
- Triage and signpost patients to appropriate services within their locality.
- Provide data that analyses the impact of mental health provision within the Integrated Discharge Service.
- Registered Nurse (Mental Health) with current NMC Registration.
- Diploma of Professional Studies (Nursing) or equivalent.
- Evidence of professional updating and development in the nursing care of patients in an acute or community setting.
- A minimum of two years post-registration mental health nursing experience.
- Skills in assessing, planning, and evaluating mental health nursing interventions.
- Knowledge of key areas of liaison psychiatric nursing practice, including the psychological effects of physical illness.
