The post holder will provide proactive and efficient case management for adult mental health patients across all ELHT hospital sites. They will work to support patients' onward and ongoing care plans, promoting timely access to health, mental health, and social care resources for safe and effective discharge.
Main Duties- Provide clinical advice and guidance to professionals within Integrated Discharge Service.
- Support patients with psychological and mild to moderate mental health needs associated with long-term physical conditions.
- Conduct assessments required for discharge.
- Triage and signpost patients to the appropriate service within their locality.
- Develop robust care plans in conjunction with the patient and wider multidisciplinary teams.
- Provide data that analyses the impact of mental health provision within the Integrated Discharge Service.

Job Responsibilities- Utilize tools such as 6CT, PHT9, MoCA, DASS21, Trusted Assessment Document, LAS, and MCA.
- Set realistic and achievable goals during assessments.
- Assess emotional and psychological needs associated with managing long-term conditions.
- Implement the Patient Actuation Model for future interventions.
- Conduct risk assessments for suicidality, risk to self or others, and intervene accordingly.
- Document visit/intervention plans on EMIS and detailed assessments in patient notes.
- Attend ward-based discharge planning meetings and best interests meetings.
- Senior and Network Management Team.
- Primary Care, LSCFT, GPs.
- Trust staff and general public/External stakeholder/commissioner organizations.
- Registered Nurse (Mental Health) with current NMC Registration.
- Diploma of Professional Studies (Nursing) or equivalent.
- Evidence of professional updating and development in acute or community nursing settings.
- A minimum of two years post-registration mental health nursing experience.
- Skills in assessing, planning, and evaluating mental health nursing interventions.
- Knowledge of key areas of liaison psychiatric nursing practice, including the psychological effects of physical illness.
- Knowledge of Common Law and issues relating to the Mental Health Act (1983).
