Job Summary

The post holder will provide proactive and efficient case management for adult mental health patients across all ELHT hospital sites. They will work to support patients' onward and ongoing care plans, promoting timely access to health, mental health, and social care resources for safe and effective discharge.

Main Duties
  • Provide clinical advice and guidance to professionals within Integrated Discharge Service.
  • Support patients with psychological and mild to moderate mental health needs associated with long-term physical conditions.
  • Conduct assessments required for discharge and triage patients to the appropriate service within their locality.
  • Develop robust care plans in conjunction with patients and wider multidisciplinary teams.
  • Provide data that analyses the impact of mental health provision within the Integrated Discharge Service.

Job Responsibilities
  • Utilize tools such as 6CT, PHT9, MoCA, DASS21, Trusted Assessment Document, LAS, and MCA to assess patients' needs.
  • Set realistic and achievable goals, considering emotional and psychological needs associated with patients' ability to manage their own long-term conditions.
  • Implement the Patient Actuation Model and conduct risk assessments to ascertain suicidality, risk to self or others, and discuss with GPs as necessary.
  • Document visit/intervention plans on EMIS and maintain detailed assessments and summaries in patient notes.
  • Attend ward-based discharge planning meetings and best interests meetings, discussing future care needs with other professionals.
