We are seeking a skilled and compassionate Community Care Specialist to join our team at Advantage Healthcare Limited. As a Community Care Specialist, you will play a vital role in delivering high-quality care to adults, children, and young people with varying conditions.

Key Responsibilities:

  • Assess and review individual care packages to ensure the best possible outcomes for our clients.
  • Provide practical training to care workers and clinical supervision to ensure the highest standards of care.
  • Work collaboratively with healthcare professionals, including ICBs, Case Management, Social Services, and private and NHS hospitals.
  • Participate in our paid phone support On Call Rota on a rotational basis.

Requirements:

  • Registered Nurse with experience in ICU/HDU or complex care.
  • Strong clinical knowledge and expertise in community care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.

What We Offer:

  • Competitive salary of £43,250 - £45,000.
