Care Support Specialist

Job Summary:
We are seeking a skilled and compassionate individual to join our team as a Care Support Specialist. As a key member of our Leaving and Aftercare Teams, you will play a vital role in supporting young people who are leaving care, helping them to maximize their life opportunities and reach their full potential.
Key Responsibilities:
- Provide practical, personal, and emotional support to young people, maintaining and promoting their health, physical, emotional development, and wellbeing.
- Support and advise young people leaving care in key areas, including health, benefits, education, training, employment, accommodation/housing, and personal well-being.
- Facilitate the assessment of need, engaging with ongoing monitoring and reassessment of need over time.
- Identify and support young people to overcome barriers to learning.
- Enable and empower young people to make decisions in their lives and to self-advocate.
- Advocate on behalf of young people who do not have the skills to do this.
- Facilitate the construction of a Pathway Plan, from the assessment which establishes a viable response to established need over time.
- Develop Personal Action Plans with young people.
- Encourage young people to fully engage with developing their individual plans, and to participate in the development of the service as a whole.
- Work alongside colleagues, both within social services and from other agencies, and engage with significant people in the young person's life.
- Support young people through times of major transitions from childhood to adulthood.
- Liaise with others involved to ensure that assessed needs are met within the agreed plans for the child or young person in a timely way.
- Build up effective interagency networks and have an understanding of voluntary and statutory agencies, as well as an awareness of opportunities within the local community.
- Maintain records and write reports as required in accordance with directorate policy and procedures.
- Carry out a range of practical tasks to assist young people with both planned and unplanned activities.
- Attend meetings as required by the line manager, e.g., reviews, planning meetings, etc.
- Maintain an awareness of relevant information and resources that may assist children, young people, and their carers.
- Keep abreast of practice developments through reading and attending training courses.
- Aware of Essex Child Protection Guidelines.
- Carry out any other appropriate duties that may be required commensurate with the grade of the post.
About You:
We are looking for an individual with experience of working in a supportive relationship with young people, and who has a creative approach to working with young people, including young parents. You will have previous work experience in either a paid or voluntary capacity working with children, young people, and families, and experience in collaborating with other professionals and agencies for the benefit of the client.
