Job Title: Community Health Nurse

We are seeking a skilled and dedicated Community Health Nurse to join our team at County Durham & Darlington NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality respiratory care to patients in their own homes and in community settings.

About the Role

This is an exciting opportunity to work as part of a dynamic team, providing holistic care to patients with complex respiratory needs. You will work closely with our specialist respiratory nursing team to assess, plan, and deliver care that meets the individual needs of each patient.

Key Responsibilities
  • Assess and manage the health needs of patients with complex respiratory conditions
  • Develop and implement individualized care plans to meet the needs of patients
  • Work collaboratively with other healthcare professionals to provide comprehensive care
  • Conduct home visits and provide respiratory care in community settings
  • Monitor and evaluate patient outcomes, making adjustments to care plans as needed
Requirements
  • Nursing Midwifery Council (NMC) registration
  • Experience working in a community setting
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
