We are seeking a highly skilled and motivated Clinical Coder/Summariser/Admin Support Specialist to join our team at Clement Road Medical Centre.
Key Responsibilities- Summarise and add SNOMED codes for relevant information from patients' clinical records into our EMIS system.
- Ensure diagnosis and investigations are reflected accurately on patients' medical records.
- Organise new patient records and letters in preparation for summarising.
- Review medical records and produce an accurate summary of patients' medical history, including identifying higher-risk areas that require incoming correspondence to the practice.
- Accurately code relevant diagnosis/procedure/information in patients' records and decide whether to send on to a GP.
- Ensure follow-up actions/tests are brought to the attention of relevant staff.
- Assist GPs with the flow of correspondence that comes into the practice, including hospital letters, reports, and other patient communications.
- Identify documents to send to the Clinical Team for review, such as medication changes and hospital results.
- Scan patient-related documents onto their medical records using codes agreed by the clinical team.
- Ensure incoming letters are scanned onto the patient record.
- Capture all QOF-related data.
- Ensure all clinical codes are current.
- Follow practice protocols to ensure consistent and high-quality care is provided to our patients.
- Action referrals on Choose and Book as recommended by the clinical team.
- Provide admin support to the rest of the admin team as required.
- Completed medical terminology course.
- Experience with using EMIS and Docman.
- English literacy and numeracy skills to GCSE level C or above.
- Experience in actioning referrals on the Choose and Book system.
- Experience working in General Practice.
- Proficient in using Microsoft Word and basic IT skills.
