About the Role

We are seeking an experienced and skilled Patient Transfer Facilitator to join our Somerset Hub for Coordinating Care team. As a key member of our team, you will play a vital role in facilitating smooth referrals and admissions for patients to community hospital beds within the Primary Care Trust.

Key Responsibilities

You will work closely with our clinicians to provide a single point of contact for information on patients to be transferred or admitted to community hospital beds. This will involve coordinating with patients, healthcare professionals, and other stakeholders to ensure seamless transitions and efficient referrals.

You will also be responsible for supporting our clinicians in providing an efficient referral process, including booking transport and liaising with patients. Additionally, you will work across multiple computer systems, coordinate telephone calls, and complete a range of reports.
