Position Overview

We are excited to present an opportunity to become a vital member of our dedicated clinical pharmacy team. The role involves collaboration within a small yet dynamic Primary Care Network (PCN) that encompasses multiple practices.

As a Clinical Pharmacy Specialist, you will join a team of experienced pharmacists and pharmacy technicians, delivering exceptional pharmaceutical care to our patients and supporting the practice teams. You will receive comprehensive support from your colleagues and participate in regular supervision and team meetings.

Key Responsibilities

Your responsibilities will include working across practices to execute our pharmacy work plan, which encompasses:

  • Implementation of Integrated Improvement Framework (IIF) and Quality and Outcomes Framework (QOF)
  • Monitoring of high-risk medications
  • Conducting thorough medication reconciliation
  • Offering expert clinical advice on medications
  • Performing structured clinical assessments to manage complex medication needs
  • Providing specialized knowledge in clinical medication management
  • Conducting face-to-face medication reviews
  • Managing chronic conditions through direct patient consultations

While a prescribing qualification and/or advanced practice certification is advantageous, it is not a prerequisite.

Job Requirements

Candidates should possess at least two years of post-graduation experience and be prepared to operate within their professional scope, collaborating with a multidisciplinary team in a Primary Care Network. You will be guided by a designated GP who will assist in your development and mentorship. Completion of the CPPE pathway is expected.

The successful candidate will provide primary support to general practice staff regarding prescription and medication inquiries. Responsibilities will include assisting with the repeat prescription system, addressing acute prescription requests, and ensuring safe prescribing practices through effective medication reconciliation.

This role is crucial for enhancing the quality of care and operational efficiency, requiring a strong commitment to delivering excellent service within general practice.

Qualifications

Essential

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.

Desirable

  • Independent prescribing qualification.
  • Advanced practice certificate.

Experience

Essential

  • Minimum of 2 years of experience as a pharmacist.
  • Familiarity with common acute and chronic conditions encountered in general practice.
  • Ability to adhere to legal, ethical, and professional standards.
  • Involvement of patients in decisions regarding their prescribed medications, ensuring adherence to NICE guidelines.

Desirable

  • Experience in a General Practice setting.
  • Experience working within a PCN.

Knowledge and Skills

Essential

  • Ability to prioritize effectively when problem-solving and recognize when to refer to senior colleagues or GPs.
  • Commitment to following legal, ethical, and professional guidelines.

Leadership and Management

Essential

  • Understanding of the pharmacy role in governance and its implementation in the workplace.
  • Ability to engage with Patient Participation Groups (PPGs) and involve them in the development of the role and practices.
  • Capability to enhance quality within service limitations.
  • Ability to set and achieve goals while promoting diversity and equality in management practices.

Management Responsibilities
