Position Overview

We are looking for a Lead for Ageing Well Initiatives who will oversee the strategic direction of comprehensive Ageing Well programs across the region, focusing on areas such as Frailty, Palliative Care, End of Life, and Enhanced Health in Care Homes.

These initiatives aim to fulfill the objectives set forth in national health strategies and local health plans.

Your role will involve the formulation and execution of Ageing Well strategies, designed to unify various service providers within the integrated care framework to improve proactive and preventive care models in the community. This will enhance integration and collaboration, ultimately leading to better patient outcomes and optimal use of resources. Furthermore, these strategies will aid in achieving operational targets and addressing variations in quality and patient experiences. The Ageing Well program manager will coordinate individual projects within the program to ensure the successful realization of agreed outcomes and benefits.

You will lead a dedicated team and also provide guidance to provider staff and resources associated with the programs.

A key aspect of your role will be to foster collaborative partnerships among stakeholders in health and social care.

Key Responsibilities

  • Ensure the successful delivery of Ageing Well programs and project objectives, providing assurance to senior management within the Integrated Community Care framework.
  • Chair the Programme Steering Group, utilizing governance structures to monitor and manage the work program effectively, ensuring that necessary changes are implemented.
  • Engage proactively with regional and national teams to identify emerging guidance and policy changes that may affect service delivery, updating stakeholders accordingly.
  • Establish priorities and critical actions, developing comprehensive program plans that align with the objectives of the initiatives.
  • Ensure that work programs are interconnected with other initiatives within the Integrated Community Care framework.
  • Manage interdependencies and address any conflicting issues, escalating unresolved matters as necessary.
  • Produce high-quality business cases aimed at achieving the objectives of the programs, including financial targets, and seek necessary approvals through established governance processes.

Qualifications and Experience

  • Senior-level experience in a complex, multi-professional organization.
