We are seeking a skilled and compassionate Field Care Supervisor to join our West Essex team. As a Trusted Assessor and Support Coordinator, you will play a vital role in engaging with customers to maximize their independence and achieve individualized goals.

  • Carry out initial assessments, outcome-focused support planning, and subsequent customer reviews in various settings.
  • Work closely with our integrated team of health and social care workers and external health professionals to maximize support available to customers.
  • Develop and maintain a comprehensive knowledge of local resources to provide effective support.

  • Hold a QCF Diploma or NVQ Level 2 in a relevant area, with a willingness to obtain Level 3.
  • Experience of working in care, with a strong understanding of customer needs and support planning.
  • A driving license and use of a personal vehicle for work purposes.
  • Confidence in traveling around the Essex area and working independently.
  • An enhanced DBS check, which can be arranged through ECL if needed.
  • Exceptional communication skills and the ability to build strong relationships with stakeholders.
  • Solid IT skills and the ability to manage your own time effectively.
