We are seeking a dedicated Mental Health Practitioner to join our Assertive Outreach team in Weymouth, covering Dorset County. As a Care Coordinator, you will work with clients who have severe and enduring mental illness, providing person-centered support to aid their holistic recovery.
Main Duties- Provide targeted support to a caseload of patients, applying philosophies of engagement and recovery in your work.
- Conduct regular clinics, including depot and Olanzapine administration, and prioritize risk assessment.
- Work collaboratively with a multi-disciplinary team, including Consultant Psychiatrists, Psychologists, and Occupational Therapists.
To be successful in this role, you will need:
- Recent experience working in a mental health environment.
- Excellent communication, organizational, and prioritization skills.
- A valid driving license and use of a car for patient assessments.
We provide comprehensive structured management supervision, management support, and peer support. We also offer opportunities for professional development, including additional learning opportunities such as NMP, ACP, and psychological therapies.
