About the Role

Navigo Health and Social Care CIC is seeking a highly motivated and compassionate Support Worker to join our Acute Older Peoples Mental Health Service team. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with mental health needs.

Key Responsibilities

  • Deliver individualized care and support plans to service users, working closely with Care Coordinators and other healthcare professionals.
  • Provide holistic care, taking into account the psychosocial and physical healthcare needs of service users.
  • Assist with daily living activities, including personal care, and promote independence and dignity.
  • Supervise and support junior staff, providing guidance and mentorship as needed.
  • Work collaboratively with the multidisciplinary team to ensure seamless care and support.
  • Facilitate community and hospital appointments, as required.
  • Monitor service user progress, identifying early signs of relapse and alerting the relevant staff.
  • Develop and implement crisis/contingency plans and advanced statements, as per local policy guidelines.
  • Maintain accurate and up-to-date records, adhering to existing procedures.

Person Specification

  • NVQ Level III or Certificate in Community Mental Health, or equivalent level of experience.
  • Experience of working with individuals with mental health needs.
  • Basic IT knowledge and ability to use SystmOne.
  • First Aid training, desirable.
  • Flexibility to meet service needs, commitment to person-centered care, and strong team player skills.
