Key Responsibilities- Undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, and dealing with queries, paperwork, reports, and correspondence in a timely fashion.
- Make professional, autonomous decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation.
- Assess the healthcare needs of patients with undifferentiated and undiagnosed problems, identifying potential health risks and developing effective care plans.
- Screen patients for disease risk factors and early signs of illness, providing health education and promoting healthy lifestyle choices.
- Develop and implement care plans in consultation with patients and in line with current practice disease management protocols.
- Provide health education to patients, promoting healthy lifestyle choices and empowering them to take control of their health.
- Record clear consultation notes to agreed standards, ensuring accurate and comprehensive patient records.
- Collect data for audit purposes and comply with Quality and Outcomes Framework (QOF) requirements, contributing to the improvement of patient care and outcomes.
- Deliver enhanced services to patients, including routine check-ups, health screenings, and disease management programs.
- Perform all general duties and responsibilities associated with a GP working within primary care, including administrative tasks and communication with patients and healthcare professionals.
