Position Overview

Symphony Healthcare Services Limited is seeking a dedicated Clinical Nurse Specialist to enhance our Nursing Team. This role involves a commitment of 15 hours per week on a fixed-term contract.

Compensation

The salary range for this position is £31,000 - £40,000 per annum pro rata, based on experience, with an NHS or NEST Pension available.

Key Responsibilities

  • Collaborate with the entire practice team to ensure effective management of patient care and needs.
  • Oversee the nursing care of patients with chronic conditions, utilizing recall systems tailored to practice requirements.
  • Contribute to the formulation of policies, protocols, and guidelines within the practice.
  • Assist the practice team in implementing new initiatives for chronic disease management services.
  • Promote the development of integrated services within Primary Care, fostering multidisciplinary teamwork.
  • Identify opportunities for collaboration with the Primary Health Care team.
  • Assess, plan, implement, and evaluate health promotion programs to enhance well-being and prevent health issues.
  • Develop and evaluate individualized treatment plans for patients with known long-term conditions.
  • Prioritize health concerns and intervene appropriately in complex or urgent situations, including initiating emergency care.
  • Utilize health promotion opportunities, such as new patient assessments.
  • Provide guidance on prescribed and over-the-counter medications, including potential side effects and interactions.

Clinical Skills Required

The role may involve various clinical skills, including but not limited to:

  • Wound care and general treatment room procedures
  • Urinalysis
  • Spirometry
  • Contraceptive and sexual health advice
  • Cytology
  • Warfarin monitoring
  • Coil removal assistance
  • Thyroxine register maintenance

Chronic Disease Management Skills

Skills in managing chronic diseases may include:

Qualifications

Essential
