About the Role:

We are seeking two compassionate and proactive individuals to take a lead in providing person-centered care and support to our service users. As a key member of our team, you will work closely with the Extra Care Registered Manager to ensure an empowering service that meets the needs of our clients and their families.

Key Responsibilities:

  • Provide support with personal care and domestic tasks
  • Support individuals to access work, education, and social opportunities
  • Work in a flexible manner, on a roster basis, including evenings, weekends, and sleep-ins

Requirements:

  • Previous practitioner experience in working with vulnerable adults
  • Ability to work in a team and provide person-centered care
